Datasets / HCPCS Level II — Healthcare Common Procedure Coding System

Public domain9k rowsReport an issue

Meridian maintains this dataset from the Centers for Medicare & Medicaid Services’ public files, and holds the cms namespace until the Centers for Medicare & Medicaid Services claims it.

Two parts of CMS's file are not CMS's own work and are left out. The dental codes (the D series) are the American Dental Association's Current Dental Terminology (CDT), and CPT is the American Medical Association's: no CPT code appears, the cross-references CMS points at CPT codes are dropped, and where one of CMS's descriptions cites a CPT code by number, the number reads [CPT code] and text_source is cms_cpt_redacted.

The list keeps terminated codes. A code can be used on a date when its added_date is on or before that date and its termination_date, the last date it may be used, is empty or on or after it. Modifiers are not included.

The Centers for Medicare & Medicaid Services maintains HCPCS Level II, and here each row is one of its alpha-numeric billing codes.[1][2] Each HCPCS Level II code is a single letter followed by four digits.[1]

Background

How HCPCS Level II relates to CPT (Level I)

HCPCS is divided into two main subsystems, Level I and Level II.[1] HCPCS Level I is Current Procedural Terminology (CPT), a numeric coding system maintained by the American Medical Association.[1] CPT codes are used primarily to identify medical services and procedures furnished by physicians and other health care professionals.[1]

HCPCS Level II codes were established so providers and suppliers can submit claims for services, supplies and equipment that CPT codes do not identify.[1][3] A HCPCS Level II code, also known as an alpha-numeric code, is a single letter followed by four numeric digits.[1]

Who uses it

Medicare and other insurers cover a variety of services, supplies and equipment that CPT codes do not identify, and HCPCS Level II codes let providers and suppliers bill for them.[1] HCPCS Level II is a system for identifying items and certain services, not a methodology for making coverage or payment determinations.[3] The existence of a HCPCS Level II code does not, of itself, determine coverage or non-coverage for an item or service.[3] Read a code as the name of an item, not as a promise that anyone will pay for it. (Meridian's view)

A HCPCS termination date is the last date for which a code may be used by Medicare providers.[4]

Anyone may submit a request to modify the HCPCS Level II national code set.[1][3]

Where it comes from

The HCPCS Level II coding system began in the 1980s.[1]

HCPCS once had a third level of local codes, developed by state Medicaid agencies, Medicare contractors and private insurers for use in specific programs and jurisdictions.[5] The use of HCPCS Level III codes was discontinued on December 31, 2003.[5]
